Hours: 9:30am-8pm, 7am-5:30pm


Role Purpose:

The Pharmacist is responsible for assisting the Director of Pharmacy in the delivery of pharmaceutical care. Duties include, but are not limited to: order review and entry, dispensing, drug therapy monitoring, supervision of technicians, ordering of pharmaceuticals, and participation in departmental and hospital future development.

    • Medication use functions: *Verifies medication orders by reviewing for accuracy of drug, dose, frequency, route of administration, duration, clinical appropriateness (reason for use) and dispensing location in hospital electronic health record. *Monitors drug therapy regimens for contraindications, therapeutic duplications, drug-drug interactions, drug-food interactions, drug-disease interactions, drug-laboratory interactions, and allergies. * Contacts prescribers regarding unclear or problem medication orders * Receives verbal and written medication orders from licensed practitioners * Obtains medication histories from patients, caregivers, and healthcare providers
    • Medication therapy management functions: *Recommends evidence-based medication therapy regimens and monitoring plans. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed * Assists with pharmacokinetics consult service and dosing per organizational protocol *Monitors patient laboratory indices and makes appropriate dosing changes when necessary *Recommends conversion of intravenous to oral therapy when appropriate
    • Dispensing of medications/IV solutions: *Accurately prepares medication and/or IV therapy doses for patients *Fills outpatient prescriptions for Hospice, Employee Health or discharged patients accurately and maintains appropriate record keeping *Assumes responsibility for technician tasks to include: checking stock selection for refilling automated dispensing cabinets, physician office products, and preparation of IVPBs/LVPs *If needed can perform all functions of the pharmacy technician
    • Control of medications: *Actively assesses the security of medication areas *Actively assesses the compliance of hospital personnel with established policies and procedures and state and federal laws including record keeping *Maintains established procedures for quality control and disposal of unusable medications and hazardous waste *Issues controlled substances and controlled substance records to patient care areas and maintains records as required by law and pharmacy policies and procedures *Reviews controlled substances records for discrepancies/errors and reports discrepancies to director
    • Patient Safety: *Detects, assesses, and reports suspected adverse drug reactions or medication errors *Participates in the development, implementation, and maintenance of medication-use safety initiatives *Adheres to safe medication-use practices according to established policies and procedures *Performs medication identification when necessary *Performs medication reconciliation activities
    • Organizational/Departmental Standards: *Attends pharmacy staff meetings *Attends and participates in other programs, committees, meetings, and functions required by the health system or pharmacy *Provides courteous, cooperative, and timely service to patients, visitors, and staff *Assists coworkers in tasks, as time permits *Performance demonstrates efforts to improve patient satisfaction, lower cost, and improve quality *Demonstrates the values and behaviors of the organization *Supports the health system's mission, vision and values *Fulfills on-call obligations per prearranged schedule *Adapts positively to changes in workload and/or work schedule *Performs work completely and accurately with attention to detail *Accepts and supports change and new ideas *Acquires and applies skills needed to achieve quality outcomes *Responds to stressful situations in a calm, rational manner *Assists technicians in monitoring recall notices and informs Director or designee when appropriate, removes product for quarantine and return, re-orders product if available or creates back-order list for future procurement, places copy of recall notice in Recall file *Assists in the preparation of purchase requests against current and projected needs for pharmaceuticals and supplies *In the absence of the Director of Pharmacy, assumes active responsibility of the department. Follows established procedures and ensures compliance with all applicable laws and regulations.


Minimum Qualifications:
  • Bachelor's Degree in Pharmacy or PharmD
  • State of Missouri Licensure
Preferred Qualifications:
  • 1 year hospital experience
  • Disease specific medication therapy management encouraged. For example: anticoagulation, diabetes, asthma are encouraged.
