Role Purpose:
The Palliative Care Nurse Practitioner works in collaboration with the Palliative Care Consultation Service Medical Director and other interdisciplinary team members and is accountable for providing specialty palliative care to patients and their families in both consultative and co-management roles without direct supervision. He or she also assists with staff and community education, and participates in hospital and health system initiatives pertinent to the field, including work on primary palliative care, program development, and hospital committees.
Specific required competencies include:
    • Sound knowledge of complex disease states
    • Ability to synthesize multiple sources of data into an accurate understanding of the patient's disease status and trajectory
    • Expertise in empathic communication with patients and their families as well as fellow health care providers and hospital / health system staff and administration
    • Ability to form and maintain a therapeutic relationship with the patient and family as well as evoke and illustrate the patient's story of his or her illness
    • Appreciation and application of principles of patient- and family-centered care
    • Promotion of the shared decision-making model of clinical decision-making
    • Facilitating and leading family meetings and goals-of-care discussions; identifying conflict, negotiating goals and plan of care, providing professional recommendations, executing an immediate plan of care, and creating or maintaining an advance care plan
    • Evaluation, diagnosis, and management of refractory symptomatology, including gathering history, performing physical exam, and diagnostic evaluation as needed
    • Providing whole person care including assessment and identification of suffering and dysfunction in the existential, emotional, and social realms
    • Coordinating care through timely communication and resource utilization
    • Participation in the development of clinical practice guidelines and standards in support of quality palliative care
  • Capacity for self-motivated adult learning in order to achieve and maintain expertise in the field of specialty palliative care
  • Use of evidence-based clinical-decision support tools, as well as medical and nursing literature, to improve individual patient care as well as the delivery of palliative care to populations in need
  • Maintaining a knowledge base regarding health care delivery systems and health care financing arrangements, especially in reference to (1) access to and delivery of palliative care, (2) long term care, and (3) hospice as a defined insurance benefit
  • Appropriate consultation with the Medical Director and other physicians when needed


Minimum Qualifications:
  • Master's Degree in Nursing (with appropriate post-master's certificate if needed) or Doctor of Nursing Practice Degree
  • Certification allowing care of adults age 18 and above
  • State of Wisconsin RN licensure, State of Wisconsin Advanced Practice Nurse Prescriber (APNP) credential, national certification (AANP, ANCC, NCB/PNP, or NCC) and DEA license
  • Advanced Certified Hospice and Palliative Nurse (ACHPN), if not already obtained, within two years of hire.
  • Two years of clinical experience in hospice, geriatrics, oncology and/or primary care.
  • Obtainment of medical staff privileges
Formal training and/or 2 years of experience in specialty palliative care is strongly preferred, though all otherwise qualified applicants are encouraged to apply.
